Santa Rosa Community Health - 0336
At Santa Rosa Community Health, we believe healthcare is a fundamental human right—everyone should have the opportunity for a full and healthy life.
Every day, we open the doors of our eight clinics in Sonoma County to care for patients in a way that honors them and meets their full health care needs. We welcome everyone with compassion and respect for their culture, language, identity, and values—no matter what.
Since 2010, SRCH has provided comprehensive primary care services to nearly 1/3 of persons at risk for or living with HIV in Sonoma County. The majority of our patients have low or very low incomes.
When you support SRCH, you are ensuring that all these services are available to everyone who needs them, regardless of their ability to pay:
--Anonymous & confidential testing for HIV & hepatitis
--HIV primary care & specialty consultation
--HIV prevention, including PrEP and nPEP
--Medical case management
--Clinical pharmacy support and help with medication
--Mental health assessment & support
--Nutritional therapy & education
--AIDS Drug Assistance Program (ADAP) enrollment
--Health education & risk reduction counseling
Thank you!
